I'm a member of OpenEMR team in Matrix Israel.
Our team is working on adding RTL capabilities, improving i18n. We are busy with Hebrew translation for OpenEMR.
Our patch to support RTL languages is included in 4.3_dev branch.
I'm a PHP and MySQL developer, maintainer of Typo3 Hebrew translation.
