- Layout Based Forms, LBF: Built in functionality to create forms within OpenEMR.
- Nation Notes: Medical Record Template engine with a WYSIWYG editor, which is built on top of the above Layout Based Forms engine.
- Xml Form Generator: This is a xml-based tool for creating new forms for OpenEMR.
- Formscript: This is a perl-based tool for creating new forms for OpenEMR.
